Our survey only takes about 4 - 8 minutes to complete and is designed to pass 80% - 90% of employees.  This translates to an "employee friendly" experience.

Employee Packet

 

A personalized packet is sent to each employee's home in a co-branded, 9 x 12 flat envelope.  This 4-page document includes:

 

1. Project Introduction

2. Instructions and Contact Information

3. Dependent Confirmation Form

4. Dependent Eligibility Reference Guide

 

The Project Introduction section provides the background and the rationale for conducting the audit.  In general, the rationale is based on compliance and controlling healthcare costs.

 

The Survey Instructions and Contact Information section provide directions and phone, fax, "snail mail" and email information.  An important part of the directions includes a unique survey code which is created for each employee.   Each caller must enter their unique survey code during the registration process.

 

The Dependent Confirmation Form is pre-populated with each dependent's name.   At the bottom of the form is a signature line along with "legal" information.   This form is only required when employees are requested to submit supporting documentation or when the employee seeks to remove one or more dependents (as this form provides an audit trail).

 

The Dependent Eligibility Reference Guide includes a list of the various types of dependents that are eligible for benefits and the supporting documentation that would be required for each dependent type in the event proof is requested.  Please note all supporting documentation is shredded shortly after the project is completed.
